Parents Need to Know

To the Parents of incoming Juniors,

The Juniors will now be required to have a physical to comply with the Commonwealth of Pennsylvania State mandates. This physical should be completed as close to the four months prior to the start or after the start  of school as possible.

If your student has a sports physical (after 6/1/18) completed this will be accepted. Your physicians form will also be accepted. The physical forms were distributed to all sophomores in homeroom and also is attached.

At the time of your child's physical the state also requires the MCV4 (2nd in series) immunization after the age of 16 years old. Please have this completed as well and have them attach a copy of the shot record.

If you have a sports physical completed the PIAA form will go to the coach for tryouts and the immunization record is to be given to the nurse.

(If you don't have a Sports Physical, then print the one below.)

Please contact me if you have any questions regarding this mandate at: or 215-624-6840 x232 M/W/F.(Sept.-June)

Thank you for your cooperation in this matter,

B. Pacitti RN, CSN

School Nurse
