The PIAA COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION (CIPPE) must be completed before participation in any tryout or practice.

The first five pages are student/parent certification.  

Page 6, the actual physical, must be completed by an authorized medical examiner and dated after May 1, 2024.
