Any student trying out for a sport is required to complete Sections 1 thru 6 of the
PIAA Comprehensive Initial Pre-Participation
Physical Evaluation and submit it prior to the first day of tryouts/practice. The form must be certified by an authorized medical examiner after June 1, 2023. You can email a copy of your form to
[email protected] or bring your completed
PIAA physical with you on the first day of tryouts/practice.
