FREE PIAA SPORTS PHYSICALS
The School District of Philadelphia and the Athlete Health Organization are joining together to sponsor a free sports physical event on Saturday, June 10th at the Wells Fargo Center. This event is for high school students only. The comprehensive physical and medical screening will meet the PIAA requirement for student-athletes interested in participating in any St. Hubert sport during the 2017 - 2018 school year. This examination is NOT a substitute for a yearly physical exam performed by your Family Doctor. A packet containing the updated PIAA Comprehensive Initial Pre-Participation Physical Evaluation for each student along with a parent or guardian signature is required prior to the exam. The packet is available in the Athletics Office and must be completed and returned to the school by May 12th. If a parent signs the forms, then they do not need to accompany their student to the event. If the forms are not signed by a parent, then a parent must attend with their student. Each school will be assigned a group time slot for their students at the event. Transportation to the Wells Fargo Center may be provided if enough students sign up for the physicals. If you have any questions or need additional information, please contact the Athletics Office at 215-624-6840, Ext. 242.
