The six page PIAA COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION  (CIPPE). must be completed before participation in any sports tryout or practice. Five pages are student/parent certification.

Page 6,  the actual physical must be completed by an authorized medical evaluator.
