For Parents of Juniors,
The Juniors are required to have a physical completed to comply with Pennsylvania State mandates.
This physical should be after June 1 and is required on the first day of school.
-If your child should have a sports physical completed this will be accepted. The physical form can be the one printable below, PIAA form, or the Primary Care Physician's form.
-At the time of your child's physical the state requires the MCV4 immunization after the age of 16 years of age. Please have this completed as well and have the immunization record attached to the physical. If a PIAA form is completed please send the immunization record to the nurse and give the physical to the Coach for tryouts.
-If your child is not at the age of 16 at the time of their physical, the MCV4 meningococcal immunization will be required on the first day of senior year.

To Senior parents,
The 12th-grade students are required to have the second dose of the immunization, MCV4-meningococcal vaccine, by the first day of senior year. If they have not received this immunization after the age of 16 during their junior year physical, please have it completed as soon as possible as it is required by the first day of school. A copy of the immunization record is to be given to the nurse.
